PERBANDINGAN KORELASI ANTARA FORMULA FRIEDEWALD DAN FORMULA MARTIN DENGAN KOLESTEROL LDL

Abstract

Penentuan kadar kolesterol LDL yang tepat diperlukan untuk pedoman terapi. Pemeriksaan kolesterol LDL yang umum dilakukan di daerah adalah dengan menghitung menggunakan Formula Friedewald, namun memiliki kekurangan, sehingga diciptakan formula hitung baru yaitu Formula Martin. Penelitian ini bertujuan membandingkan korelasi kolesterol LDL hasil perhitungan formula Friedewald dan Formula Martin dengan kolesterol LDL yang diukur langsung (direk). Penelitian ini dilakukan di RSUD Lubuk Sikaping dari April-Juli 2020. Sampel penelitian adalah pasien yang melakukan pemeriksaan profil lipid yang memenuhi kriteria inklusi dan eksklusi. Pemeriksaan kolesterol total, kolesterol HDL, kolesterol LDL (direk), dan trigliserida dilakukan dengan alat kimia klinik otomatis. Kolesterol LDL juga dihitung dengan Formula Fridewald dan Formula Martin. Perbandingan korelasi antara kolesterol hasil hitung kedua formula dengan kolesterol LDL (direk) dianalisis dengan uji Pearson. Sebanyak 85 sampel dilakukan pemeriksaan dan hasil uji menunjukkan korelasi kolesterol LDL Formula Friedewald maupun kolesterol LDL Formula Martin dengan kolesterol direk sangat kuat dengan nilai masing-masing 0,868 dan 0,887. Dalam perhitungan kolesterol LDL, Formula Martin serupa dengan Formula Friedewald sehingga aplikasi klinisnya masih harus diteliti lebih lanjut.

Determination of LDL cholesterol level appropriately is important for therapy guidelines. LDL cholesterol examination that was commonly carried out in borderland is by calculating using Friedewald Formula, but it had shortcomings, so a new calculation formula is created, the Martin Formula. This study aimed to compare correlation of LDL cholesterol calculated by the Friedewald Formula and Martin Formula with LDL cholesterol measured directly. This research was conducted at the RSUD Lubuk Sikaping  from April to July 2020. The sample of this study were patients who examined lipid profiles that met inclusion and exclusion criteria. Examination of total cholesterol, HDL cholesterol, LDL cholesterol (direct), and triglycerides were carried out by  automatic clinical chemistry. LDL cholesterol was also calculated with Fridewald Formula and Martin Formula. Comparison of correlation between calculated cholesterol from the two formulas and LDL cholesterol (direct) were analyzed using the Pearson test. A total of 85 samples were examined and the test results showed correlation between Friedewald Formula LDL cholesterol and Martin Formula LDL cholesterol with direct cholesterol was very strong with values ​​of 0.868 and 0.887, respectively. In calculating LDL cholesterol, Martin Formula is similar with Friedewald Formula, so its clinical application remains to be investigated further.
